Key Responsibilities:

* Perform comprehensive physical therapy evaluations for new admissions and provide occasional long-term care evaluations as needed.

* Develop and implement individualized treatment plans.

* Supervise and provide electronic signature review for two Physical Therapy Assistants weekly.

* Collaborate with an interdisciplinary care team to ensure the best outcomes for patients.

* Maintain accurate and timely documentation according to facility and regulatory standards.

* Ensure patient safety and provide patient and caregiver education on therapy plans and exercises.
