What Youll Do as a Home Health Physical Therapist:
- Deliver skilled physical therapy to patients based on an individualized Plan of Treatment developed in coordination with the referring physician.
- Evaluate patient needs and develop rehabilitation goals to improve mobility, strength, balance, and overall physical function.
- Adjust treatment plans and techniques as needed to achieve optimal outcomes.
- Collaborate with the interdisciplinary home health teamincluding nurses, occupational therapists, speech therapists, and social workersto provide coordinated care.
- Educate patients and caregivers on exercises, safety, mobility strategies, and use of assistive devices.
- Document all visits, assessments, and progress notes accurately and in a timely manner.
- Support patients in maximizing their independence and improving quality of lifewherever they call home.
Qualifications What Youll Bring:
- Masters degree in Physical Therapy from an accredited program.
- Current PT license in the state of employment (or eligibility to obtain).
- CPR certification required.
- Strong communication, assessment, and problem-solving skills.
- Ability to work independently in a variety of home environments.
- Valid drivers license, auto insurance, and reliable transportation.
Preferred Experience (Not Required):
- 1+ year of experience in a hospital, home health, or outpatient rehab setting
- In-home therapy delivery
- Geriatric rehab, orthopedic or neurologic rehab
- Post-acute care or chronic condition management
- Working as part of a mobile or interdisciplinary care team
