Status: Full-Time
Shift: Monday-Friday, 8:00 AM - 4:30 PM or 8:30 AM - 5:00 PM
No nights, weekends, or on-call
Salary: $82,236 - $101,000 annually
Position Overview
We are seeking a dedicated Physical Therapist to join an innovative program supporting seniors in their homes, at a day center, and occasionally in nursing home settings. In this role, you will deliver high-quality evaluation and treatment for participants at acute, restorative, or maintenance levels. Approximately 20% of your responsibilities will include home care visits to ensure a holistic and coordinated approach to patient-centered care.
Key Responsibilities
Perform in-person comprehensive physical therapy assessments upon enrollment and collaborate with an interdisciplinary team to develop individualized care plans.
Evaluate, plan, and implement physical therapy interventions for participants with a range of disabilities to maximize functional independence.
Assess needs for adaptive durable medical equipment (DME), specialized footwear, and prosthetics, recommending solutions and ensuring proper fit.
Provide ongoing documentation of therapy progress in compliance with charting requirements.
Maintain a quality assurance program and adhere to safety, environmental, and infection control standards.
Direct, supervise, and evaluate Physical Therapy Assistants, including performing competencies and supporting professional development.
Provide staff training on safe body mechanics, transfers, gait patterns, and techniques to enhance activities of daily living (ADLs).
Collaborate with nursing home and assisted living staff to coordinate therapy services for participants residing there.
Qualifications
Bachelor's degree in Physical Therapy from an accredited program
Active, unencumbered physical therapy license in Delaware
One year of experience working with a frail or elderly population preferred (training available for candidates without experience)
CPR and First Aid Certification (may be required)
Valid driver's license, auto insurance, and use of personal vehicle for home visits
Ability to manage change, prioritize effectively, and work independently
Excellent written and verbal communication skills
Proficiency in Microsoft Office (Word, Excel, PowerPoint, email, and calendaring systems)
