About the Role
As a Physical Therapist, you'll provide outstanding outpatient rehab services through a collaborative care model designed to enhance patient performance and functional quality of life.
Key Responsibilities:
Perform evaluations, develop treatment plans, and establish goals
Update goals throughout the course of care
Document patient progress and communicate with referring providers
Supervise rehabilitation techs and collaborate with administrative staff
Participate in ongoing education, mentoring, and outreach efforts
Qualifications:
Graduate of an accredited Physical Therapy program
Illinois PT license (or eligibility)
Strong communication skills
Comfort working in a fast-paced, team-oriented environment
Able to lift over 25 pounds and use appropriate body mechanics
