Job Title: Home Health Physical Therapist - Full-Time

Location: Pocono Area

We are seeking motivated and compassionate Physical Therapists to join our Home Health team in the Pocono area. This is a vital role within a close-knit, supportive team, and an opportunity to make a meaningful impact on patients' quality of life. A $25,000 sign-on bonus and relocation assistance are available.

Schedule & Work Environment:

Full-time, Monday - Friday

No weekends

No holiday shifts required unless volunteering

Compensation & Benefits:

Salary based on experience

$25,000 sign-on bonus

Mileage reimbursement at 67 cents per mile for patient-related travel

Competitive bonus incentives for additional visits

Key Responsibilities:

Provide home health physical therapy services within designated areas, helping patients restore function and achieve independence

Develop and update individualized treatment plans in collaboration with patients and their families

Work closely with Rehab Managers and clinical support staff to ensure high-quality care

Engage with team members in regular communication to coordinate care and scheduling

Qualifications:

Licensed Physical Therapist with at least 2 years of PT experience; experience in home health or adult/geriatric care preferred

Proficiency in using electronic health records (experience with EPIC is a plus)

Strong communication and organizational skills

Ability to work collaboratively within a structured yet flexible home health setting
