The in-home setting could be a patient's privateresidence or in a patient's room in an independent living facility.Powerback Rehabilitation To You allows the Physical Therapist toevaluate and treat in the most functional environment which is thepatient's home. The Physical Therapist assesses the need for,develops, and delivers physical therapy programs to facilitaterehabilitation. The Physical Therapist identifies the need for andmakes referral(s) to other disciplines to address the comprehensiveneeds of patients. They implement individualized therapy programsdesigned to restore, reinforce, and enhance performance. Inaddition, the Physical Therapist instructs, educates and trainspatients and caregivers in those skills and functions essential forpromoting independence and productivity, in order to diminish orcorrect pathology. They direct activities to promote and maintainhealth. This position provides a flexible schedule and workenvironment in various settings. 1. As necessary or applicable,supervision of Physical Therapy Assistants, temporary licensees,students and support personnel in accordance with state licensurerequirements and professional standards. 2. Responsible foradherence to department and patient treatment schedule, timelyclinical and billing documentation requirements as well as workrelated meetings. 3. Attends and contributes to patient care,staffing conferences and other related meetings. 4. Makes referralsto other disciplines and agencies in order to address thecomprehensive needs of the patient. 5. Consults with other membersof the rehabilitation team to select the most appropriatetherapeutic regimen consistent with the needs and capabilities ofeach patient. 6. Adapts program and/or plan of care according tothe needs of the individual patient. 7. Promotes a safe environmentand enforces the elimination of fire and safety hazards. 8. Orderssupplies and equipment as necessary. 9. Cleans, maintains,identifies, and reports the need for repair of equipment asnecessary. 10. Puts patient service first: ensures that patientsand families receive the highest quality of service in a caring andcompassionate atmosphere which recognizes the individuals' needsand rights. 11.Screens, examines and evaluates patients, includinghistory, systems review, and application of appropriate tests andmeasures, synthesis information and interprets examination findingsin order to establish a diagnosis, identify impairments, determinethe predicted level of improvement and the time required to achieveit, identify precautions/contraindications and design and plan ofcare. Develops appropriate treatment goals and methods incollaboration with the patient and caregivers, implements thephysical therapy treatment plan, and completes all relateddocumentation and record keeping regarding these services. Servicesinclude but are not limited to application of therapeutictechniques in areas such as: * Therapeutic exercise includingaerobic capacity conditioning, balance coordination, agilitytraining, body mechanics and postural stabilization, flexibilityexercises, gait and locomotion training, neuromotor developmenttraining, relaxation, strength, power, and endurance training *Functional training * Manual therapy * Airway clearance techniques* Integumentary repair and protection * Use of electrotherapeutic,physical agent and mechanical modalities * Health and Wellness *Education on Chronic Disease Management * Falls Assessment andInterventions to reduce fall risk * Case Management in the home andcommunity 12. Designs, fabricates, fits and/or applies adaptivedevices, and trains patients in the use of adaptive devicesincluded orthotics and prosthetics. 13. Selects appropriateactivities suited to the individual's physical and mental capacity,intelligence level and interest. 14. Instructs patient in theskills and techniques required for participation in mobilityactivities and evaluates patient progress, attitudes, and behavioras related to rehabilitative potential. 15. Performs routinereexamination as needed/required to modify/progress plan oftreatment. 16. Incorporates health and wellness programming intotreatment plans for patients. 17. Performs home and communitymobility assessments and provides recommendations for safe mobilityin the home and community space. 18. Performs other related dutiesas required. Qualifications: 1. Must have a valid Physical Therapylicense in the state(s) of practice, or proof of licenseeligibility; and 2. They must have a Master's degree in PhysicalTherapy; or 3. They must have a Master's degree in a PhysicalTherapy related field in addition to a Bachelor's degree inPhysical Therapy; or 4. They must have a Bachelor's degree inPhysical Therapy plus 5 years of progressively responsible PTexperience. 5. The qualifying Physical Therapy degree earnedoutside of the U.S. must be evaluated to be the U.S. equivalent toa degree in Physical Therapy. 6. Must have a valid driver's licenseor reliable, independent transportation to and from the patients'residence Posted Salary Range: USD $55.00 - USD $63.00 /Hr.
