This position is for a Care Coordinator who supports the mission of Saving Lives, Instilling Hope and Restoring Relationships in an integrated team with patients receiving counseling, case management and medically assisted treatment. The Care Coordinator's responsibilities include, but are not limited to the following:
- Assess patient needs and barriers
- Develop and implement the treatment plan in collaboration with the patient
- Advocate on behalf of the patient for needed services and benefits
- Connect patients with appropriate resources and assist them with obtaining their needs such as transportation, medical assistance, etc.
- Provide education, modeling, and support in the areas of daily living, budgeting, money management, and medication management
- Engage patient in recovery-oriented, patient centered care in accordance with professional standards of practice
- Complete required Documentation in an appropriate and timely manner
- Ensuring compliance with HIPPA and CFR 42 part 2 regulations
- Collaboration with all members of the multidisciplinary team to enhance patient experience and continuity of care
- Community Outreach as applicable.
Education :
- Bachelors degree in one of the following fields (social work, psychology, psychiatric rehabilitation, sociology, counseling, vocational rehabilitation, human services counseling)
OR
- Licensure by the Commonwealth of Virginia as a Registered Nurse (RN)
OR
- Board of Counseling Certified Substance Abuse Counselor (CSAC) or CSAC-Assistant under supervision as defined in 18VAC115-30-10 et seq.
Experience :
- One year of substance use related direct experience providing services to individuals with a diagnosis of substance use disorder.
OR
- A minimum of one year of clinical experience working with individual with cooccurring Diagnoses of substance use disorder and mental illness.
