Pathway Caring for Children is a nonprofit mental health agency dedicated to strengthening the lives of children and families for over 50 years. Through trauma-informed care, we offer support in foster care, adoption, and mental health services across Northeast Ohio.
We're hiring a Mental Health Case Manager to support children, teens, and their families with emotional, behavioral, and social challenges. This community-based role involves working in homes, schools, and neighborhood settings to deliver mental health support and connect families to vital resources.
Key Responsibilities:
Provide individualized support in homes, schools, and community settings
Collaborate with therapists to implement client-centered treatment plans
Facilitate group sessions on social skills, conflict resolution, and coping strategies
Help clients and families access community services and support systems
Advocate for families in school, medical, and social service systems
Maintain accurate case notes and documentation for compliance and quality assurance
Qualifications:
High school diploma or GED required
Experience working with children, teens, or families preferred
Strong communication, organization, and problem-solving skills
Ability to work independently while managing multiple priorities
Must be 21+ years old
Valid driver's license, reliable transportation, and current auto insurance
Ability to pass required background checks
Schedule & Work Environment:
Flexible weekday schedule with occasional evenings to meet client needs
Work in various locations (homes, schools, community sites, and office)
Mileage reimbursement available
