Shawnee Health is seeking a Bilingual Clinical Case Manager for our Shawnee Health Murphysboro office. This position reports directly to the Clinical Case Manager Coordinator.

Responsibilities

• Assesses for social determinants related to health disparities
• Connects patients to resources to improve overall physical and emotional health outcomes
• Assists patients with completing applications for local, state, and federal benefit programs

Requirements

• Bachelor's degree in related field and additional two years Social Service related field
• Advanced level of oral fluency in English and Spanish.
• Must have excellent communication skills, both written and verbal in addition to interpersonal and organizational skills in English and Spanish.
• Excellent reading and writing skills in English and Spanish.

Starting salary: $45,968 - $48,641with an increase upon successful completion of new hire probationary period. Compensation may be negotiable and is commensurate with experience and qualifications.
