• Provide skilled physical therapy services consistent with licensure as part of an interdisciplinary treatment team.

• Education: 4 Year/Bachelors Degree, required. Graduate from an accredited PT program, required.
• Licensure/Certification/Registration: Appropriate state PT license, required.
