Provides coordinated physical therapy to patients under the supervision of a licensed Physical Therapist. Participates in the coordination of care.
Education:
Education per PTA licensure.
Minimum Work Experience:
One year experience as a Physical Therapy Assistant preferred.
Required License:
Current PTA licensure valid for the state in which services are provided. Current driver's license, auto liability, and BLS (Basic Life Support) Healthcare Provider course according to RQI program guidelines must be completed during new hire orientation prior to working in a clinical setting. Reliable transportation required.
Required Skills, Knowledge, and Abilities:
Therapy skills defined as generally accepted standards of practice. Good interpersonal skills. Knowledge of durable medical equipment. Basic computer skills.

Promotes agency philosophy and administrative policies to ensure quality of care.
Demonstrates commitment, professional growth, and competency.
Assures the continuity of care through delivery of quality patient care.
Provides effective communication to the patient, family, team members, and other health care professionals as evidenced by clinical notes, case conferences, and communication notes. Completes appropriate documentation in a timely manner.
Monitors assigned cases to ensure compliance with requirements of third party payers.
Participates in the agency's QAPI program.
Provides care using infection control measures that protect both staff and patient.
Under the direction of the RN, assist in identifying the patient's physical, psycho social, and environmental needs as evidenced by documentation.
Participates in planning and implementing care in partnership with the patient, representative (if any), caregiver(s), and with the RN. Provides services that are ordered by the physician as indicated in the plan of care.
Provides patient and caregiver education.
Participates in Agency-sponsored in-service training.
Hospital-Wide Standards
Attends/completes hospital-sponsored training programs as required by department.
Maintains licensure, certifications and meets other position requirements and performs essential functions.
Successfully completes all assigned training on or before the due date, and annual skills competency as determined by the hospital.
Assists with maintaining a safe, clean and orderly environment. Applies Just Culture principles to daily work activities.
Operates all equipment and performs all procedures/care in a safe manner.
Performs appropriate age specific and cultural sensitive care/service to population served.
Demonstrates awareness of and use of appropriate body mechanics.
Takes initiative to complete additional duties that support the function of the department/unit.
Adheres to policies related to: emergency preparedness, fire, hazardous materials and waste, medical equipment, safety, security and utilities and understands role and expected level of participation.
Applies standard precautions; complies with OSHA standards pursuant to blood borne pathogen, tuberculosis and hazardous material and waste; complies with infection control policies including isolation precautions and hand washing.
Participates in Performance Improvement initiatives.
Reports questionable situations, concerns, complaints or harassment immediately.
Ensures a safe environment by maintaining appropriate control measures.
Maintains confidentiality of all hospital and patient information at all times as observed by peers and leadership.
Performs other duties as assigned or required and is willing to help in other departments and take on additional tasks.
May be required to work weekdays and/or weekends, evenings and/or night shifts if needed to meet the needs of the department/hospital.
May be required to work on religious and/or legal holidays.
Will be required to work, as necessary, during disaster situations, i.e., before, during and after a disaster.
May work under stressful circumstances at times.
Has regular, reliable and predictable attendance and punctuality.
Adheres to hospital/department dress code, including wearing ID badge.
