Key Responsibilities:

  • Conduct comprehensive wound assessments and develop individualized treatment plans.
  • Administer advanced wound care therapies, including negative pressure wound therapy, and specialized dressing applications.
  • Monitor wound healing progress and modify care plans as clinically indicated.
  • Educate patients and caregivers on wound care techniques, prevention strategies, and self-management.
  • Deliver skilled nursing care to patients with a variety of needs, including chronic illness, post-surgical recovery, and injury rehabilitation.
  • Perform initial patient assessments and create personalized care plans.
  • Administer prescribed medications and treatments accurately and safely.
  • Collaborate with interdisciplinary care teamsincluding physicians, therapists, aides, and social workersto ensure coordinated, holistic care.
  • Facilitate new patient admissions and maintain accurate, timely documentation using an iPad and EMR system.
  • Promote patient safety and injury prevention through proactive care planning and education.
  • Participate in quality improvement initiatives and ensure compliance with clinical standards and regulatory requirements.
  • Participate in a limited number of on-call shifts each month, supported by a dedicated on-call team.
  • Maintain complete and accurate documentation of all care activities in the EMR system.

Requirements Qualifications
  • Active Iowa Registered Nurse License and (RN) degree; Bachelor's degree (BSN) preferred
  • Minimum one year of home health nursing experience preferred.
  • Strong wound, clinical and case management skills.
  • Excellent communication and interpersonal abilities.
  • Proficiency with EMR systems.
  • Valid Iowa drivers license and reliable transportation. (VNA provides mileage reimbursement)
  • Available to work Monday-Friday 8AM-5PM includes Call & Holiday rotations
  • Special consideration for wound care experience
  • Excellent communication and interpersonal skills with ability to communicate in English verbally and in writing
  • Special consideration for bilingual nurses!
  • US work authorization
  • Strong critical thinking and problem-solving skills
  • Ability to work independently and as part of a team
  • Knowledge of federal and state regulations related to home health care, preferred
  • A sincere desire to help others
