Astria Health is seeking a Physical Therapist to join our Home Health team! This is for our Sunnyside office. This position is a full time position.
What you get to do every day!
As the Physical Therapist , you will be responsible for providing Physical Therapy evaluation and treatment utilizing evidenced-based interventions. You will:
Understand and adhere to established policies and procedures
Provide physician prescribed physical therapy
Improve or minimizes residual physical disabilities of the patient
Return the individual to optimum and productive level within the patients capabilities
Participate with all other home care personnel in patient care planning
Direct and supervise personnel as required
Responsible for initial assessment, plan of care, maintenance program development and modifications and reassessments
Perform all skilled procedures as ordered by physician
Consult with physician regarding patients progress
Instruct patient and family/significant others in home programs and activities of daily living
Participate in in-service programs and presents in-service programs as assigned
Attend patient care conferences as scheduled
Prepare clinical and progress notes
Assist physician with evaluating level of function
Helps develop the plan of care and revise as necessary
Consuls with family and Agency personnel
Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges and significant change in condition in accordance with Agency defined time frame
Appropriately utilizes ICD-10 codes
Perform other duties as assigned
Collaborate effectively across organizational boundaries.
Adhere to the core values and standards of the organization.
Required Qualifications and Education
- Graduate of an accredited Physical Therapy curriculum
- Current Washington State Physical Therapist License
- Basic Life Support certification.
- Oral and written fluency in the English language.
Preferred Qualifications and Education
- One year of Home Health experience
