- Evaluate patients and create personalized treatment plans.
- Document evaluations, daily notes, progress notes, plans of care, and discharge summaries timely.
- Collaborate with rehabilitation team to coordinate treatment programs.
- Develop home programs and treatment protocols using evidence-based guidelines.
- Manage referrals and build rapport with healthcare practitioners.
- Monitor patient progress and adjust treatment plans as needed.
- Train and evaluate therapists and supervise occupational therapy assistants.
- Assist with staff orientation, equipment maintenance, and supply inventory.

- Contribute to Quality Assurance reports and program development.
- Attend department meetings and comply with policies and procedures.
- Master's or Doctorate degree in Occupational Therapy.
- Valid Iowa Occupational Therapy license.
- Compliance with professional standards and ethical guidelines.
- Current Healthcare Provider CPR certification.
- Mandatory Reporter training certification (within 30 days of hire).

- Must have a valid driver's license, auto liability insurance, and reliable transportation as driving is required.
You will work out of the Creston, Iowa location, providing care to patients in home and nursing facilities in the surrounding area, primarily serving the elderly population. You will report to the Homecare Director in Creston, working alongside 30 other staff members including RNs, Aids, and a Clinical Manager. You will handle on average 15 cases per day, typically ranging from 12 to 18 cases. Travel is required between locations and homes using your own vehicle. The Creston location serves several counties including Adair, Adams, Clark, Decatur, Ringgold, Taylor, and Union. Physical requirements include medium work exerting up to 50 pounds of force occasionally, and/or up to 20 pounds frequently. The role involves various physical activities such as climbing, balancing, stooping, kneeling, crouching, crawling, reaching, standing, walking, pushing, pulling, lifting, talking, fingering, hearing, grasping, and repetitive motions. Visual acuity is needed for analyzing data, viewing computer screens, using measurement devices, operating machines, and making general observations. Typical working hours are 7:30 AM to 4 PM with flexibility required for varying schedules and programmatic requirements. Latest appointments usually extend until 5 PM. Pay and Benefits The pay range for this position is $76413.00 - $114620.00/yr.Eligibility requirements apply to some benefits and may depend on your job classification and length of employment.
