The Case Manager provides case management services to assigned clients, providing information and resources for clients to help them manage their basic daily needs, coordinate appointments between organizations, provide education about community resources to clients and families.

Provides case management and supports Coordination and Community Living Supports (CLS) /Respite services for children and families on an assigned caseload in accordance with all applicable contractual and regulatory requirements and accreditation standards.

Duties and Responsibilities

  • Assumes primary responsibility for all persons served on assigned caseload, including intake, assessment, periodic reviews, and annual assessments. Responsible for all reports, progress notes, collateral contacts and assignments as required. Assume responsibility for all case files on assigned caseloads
  • Provide a range of case management, including social casework, engagement, mentoring, and community referral for youth ages 3 to 21 in partnership with CMH agency and other service providers, as applicable. Responsible for all documentation and reports for assigned cases
  • Participate with Clinical Supervisor and other staff in the care planning and goal setting for the growth and development of vulnerable individuals served. Coordinate the development, implementation, and evaluation of Person Centered Planning with Clinical Supervisor and Program Manager, in conjunction with funding source
  • Preserve the health and safety of the persons served in order that he/she may reside or be supported in the most integrated, independent, community setting
  • Assist individuals with activities and skill development in daily living while preserving their health and safety, including but not limited to, household tasks, communication skills, participating in regular community activities and recreation opportunities
  • Assist the family in relating to and caring for children; attend medical appointments, acquire or procure goods other than those listed as shopping and medical services. Provide a variety of personal health care services, including but not limited to: Non-medical care, transportation, and administering medications
  • Work collaboratively with referral sources, agencies, institutions, and individuals who have important roles in the clients life situation. Communicates no less than monthly with each childs mental health provider to monitor each youths progress in treatment and ensure coordination of services to the child
  • Coordinate and collaborate with the educational system, having contact with the childs school, as defined in the treatment plan
  • Assist with and coordinate services to meet individual needs of identified youth consumer and family, as well as assistance with obtaining adult Guardianship, where applicable

    Job Qualifications

    Education, Training, and Licensure/Certification

    • Bachelor degree in human service related field, Masters Degree preferred

    Experience

    • One year of experience working with individuals having disabilities, mental illness, and/or children in a residential setting

    Knowledge Skills and Abilities

    • Ability to effectively manage crisis situations and solve problems; ability to use de-escalating techniques. Excellent organization and prioritization skills; ability to utilize computer technology and various applications, including spreadsheets
    • Willingness and ability to demonstrate compassion, a non-judgmental attitude, and empathy towards persons served; ability to maintain a helping role and intervene in situations when appropriate
    • Ability to work with children and families with multiple problems and an overall ability to relate and engage with children and families in the resolution of these problems
    • Ability to facilitate and collaboratively work with the treatment team to establish and implement treatment plans, goals and behavioral interventions. Ability to develop outcomes measurements related to Person Centered Planning goals
    • Ability to read, speak and write fluent English; strong oral and written communication skills including legible handwriting; ability to write simple and complex sentences with proper use of grammar and word order. Ability to work in team relationships and communicate professionally and extensively with government regulatory personnel, staff, service provides, clients and families
    • Ability to be culturally competent and relate to multi-problem families

    Additional Work Requirements

    • Unrestricted, valid drivers license with good driving record. Reliable personal transportation suitable for safely transporting persons served; proof of current automobile insurance
