Contract Details:
- Company: Sunbelt Staffing, on behalf of a nationwide home health agency.
- Position: Full-time Home Health Physical Therapist (PT)
- Location: Canton, TX , and surrounding areas
- Productivity: 25 per week
- Schedule: Monday-Friday, 8:00 am - 5:00 pm with weekend rotation
- Compensation: Negotiable hourly rate based on territory budget and candidate experience
- Mileage: .48c visit to visit, 150-250 miles per week expected
- Benefits include medical, dental, vision, and 401k after 90 days. Weekly pay.

Job Requirements:
- Active licensure as a Physical Therapist in Texas.
- Active American Heart Association (AHA) or American Red Cross Basic Life Support (BLS) certification.
- Minimum of 1 year of experience in home health physical therapy.
- Proficiency with Homecare Homebase (HCHB) software, Oasis, and Outlook Point Care.
- Excellent communication and interpersonal skills.
- Ability to maintain productivity standards
Job Responsibilities:
- Conducting comprehensive assessments of patients' physical conditions and functional abilities in their home environments.
- Developing individualized treatment plans aimed at improving mobility, reducing pain, and enhancing overall function.
- Providing hands-on physical therapy interventions, including therapeutic exercises, manual therapy techniques, and gait training.
- Collaborating closely with interdisciplinary team members, including physicians, nurses, and occupational therapists, to ensure coordinated and holistic patient care.
- Educating patients and caregivers on proper techniques, safety measures, and home exercise programs to support ongoing progress.
- Maintaining accurate and timely documentation of patient evaluations, treatment interventions, and outcomes using electronic health record systems.
