Contract Details:

  • Company: Sunbelt Staffing, on behalf of a nationwide home health agency.
  • Position: Full-time Home Health Physical Therapist (PT)
  • Location: Charlton, MA, and surrounding areas
  • Schedule: Monday-Friday, 8:00 am - 5:00 pm, with one weekend per month on call
  • Compensation: Negotiable hourly rate based on territory budget and candidate experience, with mileage reimbursement. Benefits include medical, dental, vision, and 401k after 90 days. Weekly pay.
  • Benefits: Medical, Dental, Vision, 401k after 90 days of employment.
  • Productivity: 25 per week

Job Requirements:

  • Active licensure as a Physical Therapist in Massachusetts.
  • Active American Heart Association (AHA) or American Red Cross Basic Life Support (BLS) certification.
  • Minimum of 1 year of experience in home health physical therapy.
  • Proficiency with Homecare Homebase (HCHB) software, Oasis, and Outlook Point Care.
  • Excellent communication and interpersonal skills.
  • Ability to maintain productivity standards of 30 units per week.

Job Responsibilities:

  • Conduct thorough assessments of patients' physical conditions and functional limitations within their homes.
  • Develop individualized treatment plans tailored to each patient's needs and goals.
  • Provide skilled physical therapy interventions, including therapeutic exercises, manual therapy techniques, and gait training.
  • Collaborate closely with interdisciplinary team members to ensure coordinated and holistic patient care.
  • Educate patients and caregivers on injury prevention, proper body mechanics, and adaptive equipment use.
  • Maintain accurate and timely documentation of patient assessments, interventions, and progress using electronic health record systems.
