Full-Time Contract, Home Health Physical Therapist (PT)-Linn, MO

Sunbelt Staffing is seeking a dedicated and compassionate Physical Therapist (PT) for a full-time home health contract in Linn, MO . This is a rewarding opportunity to provide personalized, one-on-one care that helps patients regain independence in the comfort of their homes.

• Job Type: Full-Time, Home Health Physical Therapist (PT)
• Location: Linn, MO , and surrounding areas
• Schedule: Monday Friday, 8:00 AM 5:00 PM
• On Call: Typically, one weekend per month
• Productivity: 25 per week
• Compensation: Competitive hourly pay (negotiable with client based on experience & territory budget), blended pay packages available for travelers, weekly pay
• Mileage: .48c visit to visit, 150-250 miles per week expected

• Active Physical Therapist (PT) license in Missouri
• Current AHA or American Red Cross BLS certification
• Minimum 1 year of home health PT experience
• Proficiency in Oasis, Homecare Homebase (HCHB), and Outlook Point Care
• Ability to maintain a productivity standard of 30 units per week
• Strong communication and interpersonal skills

• Perform comprehensive patient assessments to evaluate physical conditions and functional limitations.
• Develop individualized treatment plans focused on pain relief, mobility improvement, and functional restoration.
• Implement evidence-based physical therapy interventions, including therapeutic exercises, gait training, and manual therapy techniques.
• Educate patients and caregivers on rehabilitation techniques and home exercise programs.
• Maintain accurate and timely documentation in electronic health records.
