Contract Details:

• Company: Sunbelt Staffing, on behalf of a nationwide home health agency.
• Position: Full-time Home Health Field Physical Therapist (PT)
• Location: Phoenix, AZ , and surrounding areas
• Schedule: Monday-Friday, 8:00 am - 5:00 pm,
• On Call: Typically, one weekend per month.
• Compensation: Negotiable hourly rate based on territory budget and candidate experience, blended pay packages available for travelers, weekly pay.
• Mileage: .48c visit to visit, 150-250 miles per week expected
• Benefits include medical, dental, vision, and 401k after 90 days.

Job Description:

• Conducting thorough assessments of patients' physical abilities and developing customized treatment plans to address their needs.
• Delivering skilled physical therapy services aimed at improving mobility, strength, balance, and functional independence.
• Collaborating with interdisciplinary team members to coordinate comprehensive care and optimize patient outcomes.
• Providing patient and caregiver education on therapeutic exercises, safety measures, and home modifications to support ongoing recovery.
• Ensuring accurate and timely documentation of patient assessments, interventions, and progress using electronic health record systems, including Homecare Homebase (HCHB).

Job Requirements:

• Active licensure as a Physical Therapist in Arizona.
• Active American Heart Association (AHA) or American Red Cross Basic Life Support (BLS) certification.
• Minimum of 1 year of experience in home health physical therapy.
• Proficiency with Homecare Homebase (HCHB) software, Oasis, and Outlook Point Care.
• Excellent communication and interpersonal skills.
• Ability to maintain productivity standards of 30 units per week.
