JOB SUMMARY: The Part-Time Rehab Coordinator serves as the primary liaison between the therapy team and administrative leadership, helping to align therapeutic interventions and goal setting with both participant needs and organizational objectives. The Rehab Coordinator will guide the department to ensure regulatory compliance, promote interdisciplinary collaboration, and support high-quality, goal-driven care.
In addition to their coordination responsibilities, the Rehab Coordinator will provide occupational therapy services on a consultative basis and offer direct support to the organization’s existing part-time OT, helping to ensure coverage, continuity, and efficiency in participant care planning.
JOB RESPONSIBILITIES:Rehab Coordinator:
- Serve as a liaison between therapy staff and administrative leadership to ensure therapy practices align with organizational goals and PACE regulatory requirements
- Support and guide therapists in setting functional, measurable goals that reflect participant needs and program standards
- Monitor therapy treatment plans to ensure services are delivered in a timely, efficient, and participant-centered manner
- Participate in interdisciplinary team (IDT) meetings to represent the therapy discipline and advocate for integrated care planning
- Audit therapy documentation for accuracy, completeness, and regulatory compliance; provide feedback and follow-up as needed
- Identify training needs and mentor therapy staff to promote quality improvement and adherence to evidence-based practices
- Collaborate with the Quality Committee to support audits, reporting, and performance improvement initiatives related to therapy
- Serves on the PACE Administrative Team and shares information with direct care staff as appropriate.
- Assist with coverage planning, caseload distribution, and coordination of therapy resources as needed to meet participant care needs
- Maintain accurate documentation in accordance with PACE, state, and federal guidelines
- Promote a collaborative, respectful, and solution-oriented culture within the therapy department
Occupational Therapist:
- Provide direct occupational therapy services on a consultative basis and assist the existing part-time OT with evaluation, treatment, and documentation duties
- Perform assessments, scheduled reassessments and unscheduled reassessments of participant condition and functional status.
- Assess participant need for equipment, supplies and assistance per Service Request system.
- Develop, revise and keep current treatment plans that are integrated with the overall plan of care.
- Communicate changes in participant status to IDT and note changes in medical records.
- Help participants develop functional strength, coordination, cognitive and balance skills to meet their occupational performance goals.
- Help participants adjust to disabilities by implementing programs to enhance self-care and home- making skills, activities of daily living and use of adaptive equipment.
- Help participants engage in sensory-motor, educational, recreational and social activities.
- Perform feeding/swallowing screens and make recommendations about food and drink textures, positioning, exercises, modified swallowing techniques and mealtime set ups and supervision.
- Make referrals for speech therapy consults when needed.
- Promote maximum independence of participants by selecting therapies according to each individual’s physical capacity, intelligence level and interest.
- Assist in the design, fabrication and application of orthoses.
- Guide participants, caregivers and staff in the selection and use of adaptive equipment and
- Suggest ways to adapt physical environments for participants with disabilities.
- Design and instruct participants, families and caregivers in home exercise programs.
- Equip participants with skills and strategies to help manage chronic pain.
- Assist in the management of home-bound participants through regular visits and telephone contacts.
- Visit participants in the home as indicated to initiate or modify adaptations and treatment.
- Participate in family conferences, team meetings, committees and work groups as required.
- Maintain a safe and clean working environment and adheres to infection control policies.
- Ensure operation of equipment by completing preventive maintenance requirements, following manufacturer’s instructions, troubleshooting malfunctions and calling for repairs.
- Participate in the Quality Improvement program.
- Participate in the coordination of a 24-hour care delivery system.

SUPERVISORY RESPONSIBILITIES: The Part-Time Rehab Coordinator serves as the primary liaison between the therapy team and administrative leadership, helping to align therapeutic interventions and goal setting with both participant needs and organizational objectives.
QUALIFICATIONS:Licensed as an Occupational Therapist by the Virginia Board of Medicine.
One or more years’ experience with a geriatric population; experience in a home health, or rehabilitative care, preferably in a PACE setting or similar disciplinary setting preferred.
Demonstrated clinical competency with an elderly and disabled population.
Strong understanding of goal-based, functional therapy planning for older adults
Excellent communication, leadership and organizational skills.
Ability to work part-time with flexibility to attend care planning meetings and respond to participant needs.
Valid driver’s license and reliable transportation.
Must work well as part of a team.
Medically cleared for communicable diseases with up-to-date immunizations before participant contact. Criminal history records check at hire.
Drug screen at hire.
WORK SCHEDULE: Part-Time. 24 hours per week during normal operating hours of 8:30 a.m. to 4:30 p.m. Monday through Friday.
