EDUCATION/CERTIFICATION AND LICENSURE: Graduate of an accredited physical therapy program Master's degree required; Doctorate (DPT) preferred Current Hawaii State license as a Physical Therapist **LICENSE REQUIRED TO APPLY Current certification in CPR at BLS/HCP level **All licenses and certifications must be current/valid at the time of submission through the end of the initial contract. B.
