Mental Health Court Case Manager

The GCMHC Case Manager assesses the nature and degree of GCMHC participant service needs. The Case Manager develops, implements and monitors the efficacy of the Court Re-Entry & Treatment Plan. The Case Manager identifies current and emerging participant needs and links participants with appropriate rehabilitative services including mental health and substance abuse treatment, housing, food, transportation and other community resources. The Case Manager will monitor participants compliance with the treatment plan by contacting treatment providers and assisting participants with non-compliance, through peer counseling and affirmations. In addition, the Case Manager serves as a liaison with the GCMHC participant, community service providers and the Court Team.

Duties, Functions and Responsibilities:

  • Develops initial GCMHC Re-Entry Plan based on psychosocial assessment and input from GCMHC Clinical Team
  • Identifies current and emerging GCMHC participant needs
  • Links GCMHC participants with appropriate rehabilitative services including mental health and substance abuse treatment, housing, food and other community resources
  • Works directly with GCMHC participants to reduce individual and systemic barriers to obtaining and maintaining resources
  • Serves as a liaison between the GCMHC participant, community service providers and the Court
  • Assists participants with the completion of referral forms/packets required by community service agencies
  • Prepares Letter of Homelessness per HUD requirements
  • Responds to community needs/requests regarding GCMHC participation
  • Works collaboratively with the Team to develop individualized Behavior Contracts
  • Prepares GCJ releasing instructions and ensures GCMHC participant is released from GCJ directly to residential treatment, transitional housing provider or LPC
  • Develops, updates and maintains resource files
  • All other duties identified by immediate supervisor

Minimum Qualifications:

  • Bachelors Level Clinician
  • Two years of case management experience
  • The GCMHC Case Manager must be a collaborative problem solver with strong communication skills

Other Requirements
Ability to travel to the courthouse Justice Center, other criminal justice system facilities or other places necessary to carry out job duties.

Possess excellent interpersonal, analytical, writing, and communication skills.

Ability to work independently as well as collaboratively in a legal services team hierarchy.

Possess a valid drivers license and have access to a vehicle in order to access clients and witnesses.
