About the Role: Access Team – Mental Health Clinician:
North Range Behavioral Health is a recipient of federal funding through the Substance Abuse and Mental Health Services Administration (SAMHSA) to be a Certified Community Behavioral Health Clinic (CCBHC). North Range Behavioral Health is now one of four centers in Colorado to join others across the nation. The CCBHC, or Access Team, is an innovative multidisciplinary team initiating immediate access to behavioral health care. The team includes clinicians, mental health counselors, and case managers working together to create a seamless and smooth front door for our large community behavioral health center in Weld County, Colorado.
Mental Health Counselor, you’ll play a key role in helping individuals navigate life’s challenges by providing compassionate, evidence-based care. You’ll conduct assessments, offer crisis support, and deliver individual, family, and group therapy, all while developing personalized, strength-based treatment plans that evolve with your clients' progress.
You’ll collaborate closely with a supportive team of clinicians, supervisors, and psychiatrists, ensuring a holistic, person-centered approach to care. You’ll also coordinate with community partners and internal resources to connect clients with the services they need to thrive.
Key Responsibilities:
- Ensure compliance with utilization, quality improvement, and corporate policies and standards.
- Provide direct services to clients as assigned by your supervisor, including assessments, goal setting, and treatment for mental health challenges.
- Conduct screenings, intakes, and assessments within established timelines, utilizing the required tools and formats.
- Develop and update strength-based treatment plans and continuing care plans, ensuring timely referrals and discharge summaries.
- Assess clients’ diagnoses and mental status and create individualized treatment plans to meet their needs.
- Provide crisis intervention and de-escalation in various settings and assess risk to ensure client safety.
- Collaborate with clinicians, law enforcement, external providers, and psychiatrists to deliver coordinated care.
- Work with clients on safety planning, symptom management, and risk education to enhance support systems and improve overall well-being.
- Utilize therapeutic interventions and behavioral skills training to help clients develop coping strategies, make informed decisions, and solve daily life challenges.
- Support and implement evidence-based practices to ensure the effectiveness of client interventions.
- Provide case management and connect clients to internal and external resources to support treatment goals.
Qualifications:
- Bachelor’s degree in psychology, social work, or a related field.
- Valid driver’s license and a driving record that meets insurability requirements.
- Successful completion of background, fingerprinting, and drug testing
- Bilingual (English/Spanish) skills are a plus.
Experience:
- 1 year of experience working with individuals with mental health challenges is required.
