Key Responsibilities:- Implement individualized treatment plans using a variety of therapy techniques and modalities
- Monitor patient progress and communicate updates or adjustments to the supervising therapist
- Document care services and patient outcomes with accuracy and clarity
- Educate patients and their caregivers about physical challenges and therapeutic goals
- Support discharge planning, including equipment recommendations and community resource referrals
- Collaborate with the healthcare team to ensure the highest quality of patient care

Qualifications and requirements: - Associate’s degree Required; Graduate of a Program in Discipline Required
- Physical Therapy Assistant in the State
- Certifications: Basic Life Support (BLS)
