Job Description

Purpose of Position:

Will provide specialized support for patients with SUD (Substance Use Disorder), and tailor a team-based plan of care that will be carried through all transitions to patient wellness

Responsibilities:

In close coordination with other ED LEADS team members, the ED LEADS Counselor may:

Supervise Peer Counselors

Engage in assessment, treatment, education, and consultative activities for patients, primarily on the inpatient unit, who may have a substance abuse problem

Develops appropriate evaluation reports and/or psycho-social addiction histories for new patients.

Documents findings, interventions, and plans in patient's medical records on the same date patients were interviewed

Minimum Qualifications

Assignment Level I

1. Valid license and current registration or valid limited permit to practice as a Mental Health Counselor issued by the New York State Education Department (NYSED).
