Occupational Therapist-Home Health

Hospital Sisters Health System (HSHS) is seeking an Occupational Therapist to join our Home Health Team. The Occupational Therapist I-Home Health evaluates all assigned patient's functional abilities and conducts individualized occupational therapy programs in a home care environment to help patients develop, regain, or maintain their ability to perform daily activities as outlined by plan of care. Responsible for documentation of evaluation, progress, discharge, and other reports as requested. Responsible for delegation and direction of all discipline-specific therapy services and provides home treatment education for patients, families, assistants and students. Performs other professional duties associated with the care of patient, development of therapy program services, and engagement and development of interdisciplinary teams.

Schedule: PRN, as needed

Education Qualifications

• Graduate (e.g., bachelors, masters, or doctorate) from an accredited Occupational Therapy program which meets the licensure requirements in the state of practice is required.

Experience Qualifications

• 6 months of Occupational Therapy experience is required.

Certifications, Licenses and Registrations

• Illinois: Licensure or Eligibility for licensure as a registered occupational therapist by the State of Illinois under the condition that they have been issued a letter of authorization from the Illinois Department of Financial & Professional Regulation (IDFPR) allowing them to practice under supervision is required.
• Wisconsin: Licensure or Eligibility for licensure as a registered occupational therapist by the Occupational Therapists Affiliated Credentialing Board in Wisconsin under the condition that they have been issued a temporary license allowing them to practice under supervision is required.
• Certified Basic Life Support (BLS) or completion in the first 90 days of employment is required.
• Valid state drivers license, current automobile insurance and reliable automobile is required.

Pay Range:

$34.21 - $51.31

A successful candidate's actual pay rate will be based on several factors including relevant experience, skills, training, certifications and education.
