About the Role:
As a Physical Therapist in Chickasha, OK, you will play a critical role in improving the physical health and functional mobility of patients across diverse age groups and conditions. Your primary objective will be to assess, plan, and implement individualized rehabilitation programs that promote recovery from injuries, surgeries, or chronic conditions, with a focus on orthopedic and geriatric populations. You will collaborate closely with physicians, occupational therapists, and other healthcare professionals to ensure comprehensive patient care. Through manual therapy techniques and evidence-based interventions, you will help patients regain strength, reduce pain, and enhance their quality of life. Ultimately, your work will empower patients to achieve optimal physical function and independence in their daily activities.
As a Physical Therapist , you will:
Perform and document an initial exam , evaluating data to identify problems and determine a diagnosis prior to intervention.
Plan, prepare, or carry out individually designed programs of physical treatment to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction in patients.
Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.
Applies concepts of infection control/universal precautions in performing patient care
Assure continuity of quality patient care delivered with appropriate documentation
Provide effective communication to patient/family, team members and other health care professionals
Promote agency philosophy and administrative policies
Maintain and complete all documentation within company stated guidelines.
Requirements
Successful completion of accredited physical therapy program and state licensure exam.
Valid Physical Therapy license
1 year of home health experience preferred
CPR certification
Valid Driver’s license
Valid Auto Insurance
HomeCare HomeBase experience preferred
Physical Demands
Must be able to stand, walk, sit, climb, crawl, squat, or kneel for extended periods.
Must be able to lift, carry, pull, or push up to 100 lbs..
