Pay is commensurate with experience within a range of $20.00 to $27.00 per hour. Full time, non-exempt, benefits eligible. Responsibilities : The primaryresponsibility of the CPST Case Manager for adults who meet the criteria forthe targeted population includes but not limited to:

  • To help clients to understand thesymptoms of their mental illness and to minimize the effect of those symptomsin all the significant areas of their lives.
  • Assist clients in learning andpracticing news skills that contribute to their recovery as specified in thetreatment plan.
  • Provide supportive counseling,solutions focused interventions; behavioral analysis to increase clientsability to function in all life domains.
  • Providing outreach and crisisintervention services to clients and prospective clients to enable timelyaccess to those services appropriate and necessary for crisis stabilizationwith the purpose of diverting the client from hospitalization and allowing theclient to maintain independent living;
  • Develop individualized crisis planwith client and review during treatment plan review time.
  • Additional responsibilities that may beassigned as circumstances warrant.
  • Regular and predictable attendance is anessential function of the job.
  • Flexibility to provide after hour coveragefor crisis services.

Qualifications : The CPST Case Manager position requires but not limited to:

  • aBA/BS degree or four years of equivalent education and/or experience working inthe human services field.
  • reliable transportation; and a valid,unrestricted driver's license
  • prior computer experience and basiccomputer fluency;
  • must be able toachieve within one's first 6 months of employment a working competency in themanagement and use of the center's electronic medical record (EMR) software asdemonstrated by timely and accurate documentation of the clinical record;
  • an ability to work independently and incooperation with others;
  • ability to exercise sound independentjudgment;
  • flexibility in assigned working hours;
  • an energetic and positive approach to therapidly evolving changes and challenges of a complex work place.
