Primary Responsibilities
The Patient Care Coordinator position is responsible for accurate and timely data entry of referral requests into the appropriate portal. They are also responsible for follow-up on all specialty referrals and coordination of care for urgent referrals. The patient care coordinator is part of the care team, working as a liaison between the patient, providers, utilization management and various departments to ensure timely access to specialty care.
Qualifications
- Two or more years of experience in customer service or one or more years of experience in a healthcare setting and prior experience in a health plan, medical group or IPA setting working with physician networks
- Ability to convey a positive and professional image while maintaining contact with focus with audience.
- Self-motivated, flexible and a team player, must have the ability to work independently, yet know when to ask questions.
- Must be well organized, have excellent phone/people skills, and be able to handle multiple tasks.
- Must have strong computer skills and effective communication skills a plus, especially in Microsoft Word, Outlook and Excel.
- Effective verbal and written communication skills; strong computer skills desirable.
- Available to work evening and Saturday.
- Bilingual English/Spanish is preferred.
