Description
  • Conduct evaluations for patients in inpatient or outpatient environments and formulate personalized treatment plans
  • Implement physical therapy techniques focusing on enhancing mobility, strength, balance, and overall function
  • Collaborate with a team of healthcare professionals, including nurses, doctors, and case managers to promote optimal patient outcomes
  • Provide education to patients and families about therapeutic objectives, discharge planning, and home-based exercises
  • Maintain comprehensive documentation of assessments, treatments, and patient progress in line with facility policies
  • Engage with interdisciplinary teams during care rounds and conferences to ensure seamless care
Education
  • Doctorate in Physical Therapy (DPT) or an equivalent degree from a recognized program
Certifications
  • State licensure as a Physical Therapist
  • CPR certification preferred
