Day-To-Day:

• Completes needs assessments regarding family reunification needs.
• Monitor clients progress on adherence to individualized treatment plans and to develop and implement treatment plan
• Provides active, intensive, case management services with focus on family reunification, including accompanying the client to the referral source if necessary.
• Interacts with Texas Department of Family and Protective Services (CPS) and provides advocacy for reunification
• Works collaboratively with schools and community mentoring youth mentoring organizations
• Conducts mother and family meetings

Qualifications

• Bachelor's Degree in a social, human services, criminal justice, behavioral science program or related field from an accredited college or university and 1 year experience.
• Must have valid Texas Driver License, clear MVR and ability to be insured.
