Position Summary : Provide goal-oriented services that assist clients and their families by locating, coordinating, and monitoring necessary care and services; insuring that they are appropriate and accessible. Work with clients and their families within the office, school, and at home. BA or BS in health and human services field required.

Essential Duties and Responsibilities : Maintain an active caseload, providing interventions as needed within area of expertise and limits of credentials.Keep regular contact (weekly or bi weekly) with the assigned social worker, attorney, school teachers and treatment team to keep updated on the current plan for each client. Gather any potential info that may be helpful. This may involve multiple contacts to care providers, daycare, bio parents or group homes.Assure that services provided are specified in the Treatment Plan and monitor progress toward treatment goals with screening tools and questionnaires.Consult and assist with community systems to facilitate linkage, referral, crisis management, advocacy, and follow up with focus on attaining treatment goals. Provide crisis management for clients, and supply linkage for interventions as appropriate. Maintain client and clinic records in accordance with applicable laws, regulations and company standards. Complete and submit progress notes with 48 hours of client contact. Maintain a high level of ethical conduct regarding client confidentiality in accordance to HIPAA laws including confidentiality, dual-relationships, and professional stature.

Job Description Assist with on boarding new client and scheduling first assessments. Link clients to other recommended services. Communicate and collaborate with treatment team. Attend and facilitate team meetings. Advocate when needed. Services may be delivered in the community by phone/telehealth or in office. We treat children and adults Provide case management and individual rehab to clients assigned to your case load. Requirements

Education/Experience : Bachelor's Degree in Psychology, Social Services, or a related field.
1-2 years minimum work experience in a related position is preferred, but not required.
