This is a Contracted PRN position.
Education: Bachelor's Degree (B.A.) from four-year college or university
Experience : One (1) to two (2) years related experience and/or training
Required License/Certifications : Current state license in Occupational Therapy
Essential Job Functions:
- Recognizes the indicators of risk for musculoskeletal and/or neurological impairments acting on the mobility. ADL's home management, vocational and leisure activities, sensory motor and psychosocial skill.
- Obtains and interprets background information relevant to the assessment process form available documentation, patient interview, family members, and other professionals.
- Establishes rapport with patient/caregiver in order to proceed efficiently with data collection.
- Selects and administers standard and non-standard measures in accordance with commonly accepted practice.
- Formulates prognosis for improvement of function and determines individual's candidacy for intervention.
- Integrates case history information along with evaluation results to differentially diagnose problems.
- Documents and communicates assessment results and recommendation for intervention within time frame established in each department.
- Makes appropriate recommendations if additional assessment is warranted (e.g. other therapist, Psychological Services, MD, etc.).
- Continues to assess the patient's response to treatment with documentation of reassessment in accordance with time frames in each department.
- Collaborates with patient/ family in establishing goals.
- Documents goals in objective, measurable, functional and time-limited terms. Reflects an interdisciplinary approach in setting of goals.
- Develops treatment plan to achieve goals and ensures treatment is within prescriptive orders.
- Demonstrates proficient performance of technical skills and operation of equipment.
- Selects a personalized intervention approach (e.g. prevention, remediation and/ or compensation with reference to functional outcomes and patient age).
- Modifies treatment in response to changes in patient's physical and emotional status.
- Collaborates and works cooperatively with physicians and other members of the multi-disciplinary health care team in formal and informal teaching of patients and/or family about his or her disease process and wellness promotion plan and discharge planning: assesses patient/family learning needs, abilities, preferences, and readiness to learn; utilizes a collaborative interdisciplinary process to provide appropriate patient/family education; utilizes available information to evaluate the effectiveness of teaching in producing the desired patient outcomes and makes adjustments in teaching as indicated.
- Coordinates discharge plan with patient, family, interdisciplinary care team, discharge site and other community resources.
- Consistently directs the support personnel based on knowledge, skills, and competencies, pracice guidelines, department policy, and patient needs.
