What Youll Do
- Conduct initial and follow-up assessments to evaluate patients in their home setting
- Create and implement individualized, evidence-based care plans
- Document patient progress and adjust plans as needed
- Instruct patients and caregivers on therapy goals, exercises, and equipment
- Work closely with nurses, social workers, and other clinicians to ensure coordinated care
- Supervise Physical Therapy Assistants in accordance with state regulations
Qualifications
- Current (or eligible) Maine licensure from the Board of Examiners in Physical Therapy
- At least 1 year of professional experience as a Physical Therapist
- Valid Maine drivers license with a clean driving record
- Ability to work independently and exercise sound clinical judgment in a home setting
