What Youll Do

• Conduct initial and follow-up assessments to evaluate patients in their home setting
• Create and implement individualized, evidence-based care plans
• Document patient progress and adjust plans as needed
• Instruct patients and caregivers on therapy goals, exercises, and equipment
• Work closely with nurses, social workers, and other clinicians to ensure coordinated care
• Supervise Physical Therapy Assistants in accordance with state regulations

Qualifications

• Current (or eligible) Maine licensure from the Board of Examiners in Physical Therapy
• At least 1 year of professional experience as a Physical Therapist
• Valid Maine drivers license with a clean driving record
• Ability to work independently and exercise sound clinical judgment in a home setting
