Evaluates, directs, plans, and provides treatments for patients with a wide variety of disabilities to help them achieve a maximum level of functional independence. Documents patient care and results of treatment appropriately in the medical record to comply with policies and procedures of the department and regulatory agencies.

Assignment Locations: 71 Hospital Ave, North Adams, MA and 725 North Street, Pittsfield, MA. Mileage will not be supplied for this need.

Non-billable orientation: 8hrs
Holiday, OT Multipliers: 1.25%

• 2 Years current OT experience
• Bachelor, Master, or Doctorate degree required
• Valid Driver License and Vehicle required
• Assignment Locations: 71 Hospital Ave, North Adams, MA and 725 North Street, Pittsfield, MA. Mileage will not be supplied for this need.
• Traveler perm address must be included in profile
• Traveler must have their own vehicle for this need
• ANY/ALL RTO MUST BE INCLUDED IN PROFILE TO BE CONSIDERED BY FACILITY

• Work History
• Skills Checklist (pertinent to position), current to 1 year
• 1 Supervisory Signed Reference (from within the past two years of employment)
• License/Certifications - Must be uploaded with profile

• BLS (AHA)
