Nurse Case Manager

RESPONSIBILITIES Coordinates the interdisciplinary treatment plan of care for patients across the healthcare continuum within a defined population. Facilitates the delivery of services, and functions as the patient advocate to identify and communicate health care needs. Works collaboratively with clinical staff, multiple University Health departments, and outside agencies in an effort to improve patient outcomes, compliance, decrease complications and support Baby Bexar program goals and objectives. EDUCATION/EXPERIENCE Graduation from an accredited school of nursing, RN is required. A minimum three years of clinical nursing experience is required, which may include service coordination, case management, quality management, or managed care experience. Experience working with OB patients preferred. Experience working with postpartum patients/families, newborns and infants highly desired. Experience working with EPIC EMR preferred. LICENSURE/CERTIFICATION Current licensure in the State of Texas as an RN is required.
