Travel Physical Therapist - Home Health

American Traveler seeks an experienced Physical Therapist for a Home Health position, requiring 1 year of experience, BLS, and Epic proficiency.

• Home health setting serving adult and geriatric patients
• Monday through Friday, day shift, 8:00 AM to 4:30 PM
• Must float to Monroe territory as needed
• Uses Epic EMR; prior Epic experience is required
• Will work primarily with orthopedic and geriatric populations
• Position includes assessment, development of care plans, and education for patients and families
• Responsibilities include working with patients recovering from hip and knee replacements or fractures
• Participation in the home exercise program and use of adaptive devices is required
• Applicant must not live within 70 miles of the facility
• Observed holidays include New Year's Day, Memorial Day, Independence Day, Labor Day, Thanksgiving, and Christmas
• Two recent professional references from a supervisor within the past 2 years are required for consideration

• Minimum 1 year of recent home health experience as a Physical Therapist
• Active BLS certification required
• State physical therapy license preferred; license required if applicable
• Experience working with geriatrics, orthopedics, adaptive devices, and OASIS documentation
• Demonstrated skills in pain assessment, plan of care development, gait belt training, and patient/family education
• Ability to perform safety and injury prevention assessments
• Epic EMR experience required
• No first-time travelers considered
