Sun River Health is currently seeking a full-time Transition of Care Coordinator to join our team in Peekskill.
Description:The Physical Therapist will be responsible for documenting the patient's treatment's current status during the evaluation, changes in condition, and other pertinent information as required. They will also provide treatment and appropriate therapeutic techniques specific to the age and condition of each patient.
Responsibilities:

- Charts are complete, containing all pertinent info; symptoms, medical hx, meds, assessment; Charts are Clear, with clear language standard terminology understood by other health professionals
- Is rarely absent, arrives punctually, works required hours; Has ability to get along with coworkers and management; Is a team player; Conveys information politely, effectively and efficiently; Shows initiative, optimism, and politeness; Has capacity to respond to changing situations and expectations; Desires to take on challenges and learn new techniques; Demonstrates a professional and well-kept appearance; Seeks constructive approaches to resolving workplace issues; Responds to all emergencies in building; Answers phone calls, messages in the EMR and emails in a timely manner; Provides off-hours and hospital coverage where contractually obligated to do so
- Individual rates on quality measures are meeting or exceeding benchmarks, or there is an upward trend toward meeting benchmarks with evidence of PDSAs or other improvement efforts/methodology to improve rates
- Goes above call of duty, e.g., volunteers to cover weekends or other sites when needed, comes to Sun River Health Community events, vaccination drives
- Contributes to a safe and secure environment by following organization's policies and procedures including but not limited to: Locks notes according to policy; Reviews labs/diagnostic imaging/referrals and documents according to policy; Responds to Telephone Encounters in a timely manner; Completes peer reviews as assigned; Completes all required trainings by the due date (e.g., Relias, ECRI, BLS); Renews licenses and DEA certificates before expiration; Follows Infection Control Policies; Reports communicable illnesses to DOH as required
- Knowledgeable of and adherent to Evidence Based protocols, treatment pathways, and institutional guidelines (e.g., prescribing patterns, vaccination protocols, opioid prescribing). Chart Reviews performed by peers (Peer Review) and/or supervisor are scored well. Refers to specialists appropriately and treats all conditions within their scope of practice
- Productivity is meeting or exceeding targets, or there is a continuous upward trend toward meeting targets
- Ensures high-quality care, respects patients dignity and confidentiality (HIPAA), provides compassionate patient care, has had no or minimal patient complaints in past year, has high Patient Satisfaction Scores; has the ability to stay on track and use time effectively
