Other duties may be assigned./ppbr /strongProvide case management and support to consumers. /strongbr /-Treat consumers with respect.br /-Provide support counseling, problem solving, contracts and limit setting.br /-Establish attainable goals with the consumer.br /-Involve consumer, agencies and others in treatment planning./pp-Collaborate with clinical team on engagement insight and needed supports./pp-Provide support counseling with mental health issues, substance use disorders, and medical conditions.br /-Assist consumer in deciding which areas they want to address.br /-Assist and monitor the group home consumers with medical interventions and hygiene needs.br /-Encourage consumers to attain the highest possible levels of independence.br /-Accept the values and lifestyles of consumers.br /-Use a flexible approach with a diverse population.br /br /strongMake assessments and provide appropriate crisis interventions and risk management as needed to maintain a calm and safe environment. /strongbr /-Assess consumers' unmet needs (psychological, physical, housing, substance use, and psychiatric).br /-Identify, assess and respond to crisis situations.br /br /strongTeach consumers adult daily living skills assisting them in all aspects of their life in the group home and supervised apartments. /strongbr /-Teach, supervise and assist consumers with doing household chores (morning and evening, laundry, room cleaning, etc.) using realistic situations./pp-Teach clients coping skills to implement symptoms management and harm reduction./pp-Provide direct alcohol and drug counseling/case management to individuals with substance use disorders./pp-Perform household duties as needed.br /-Use a step-by-step process when teaching skills.br /-Use positive reinforcement and encouragement.br /-Integrate skills training in all aspects of their treatment plan.br /-Maintain a clean, healthy and safe environment at all times./pp-Adhere to Infection Control consumers within The Center and with outside agencies (Social Security Supplemental Insurance (S.S.I.), Food Stamps, Advocates Office, Medical Doctor, etc.)./pp-Negotiate on behalf of the consumer, as needed, with landlords, courts, families, treatment team, consumers themselves.br /br /strongAssist in the development and implementation of socialization and recreational activities within the group home, in the community, and by attending Day Program groups and activities. /strongbr /-Develop professional relationship with Day Program staff.br /-Provide and maintain a home atmosphere.br /-Maintain and promote family contacts and relationships.br /br /strongTransport consumers in own vehicle or Center van to planned activities and appointments. /strongbr /-Drive carefully and abide by traffic rules.br /-Ensure that seat belts are worn by all traveling in Center or owned vehicles.br /br /strongComplete recordkeeping in accordance with Health Information Services (HIS) and other Center requirements and abide by Center confidentiality policy. /strong/pp-Complete required interagency and other necessary paperwork.br /-Complete shift activity logs.br /-Complete all required
