Case Manager

T he Case Manager is responsible for coordinating the overall interdisciplinary plan of care for patients, from admission to discharge. Serves as a liaison between the patient and healthcare personnel to maximize the quality of care and cost-effective utilization of services.

Requirements

Qualifications and Experience

  • High school diploma or equivalent
  • Minimum two years of patient service experience in a healthcare setting
  • Ability to act with integrity, professionalism, and confidentiality
  • Experience using electronic medical records systems
  • Excellent communication and interpersonal skills
  • Excellent organizational skills and attention to detail

Responsibilities include, but are not limited to the following

  • Develops and facilitates patients plan of care, specific to the patient's need
  • Maintains accurate clinical documentation of the patient's care plan in the case management system
  • Collaborate with multidisciplinary teams in acute and outpatient settings to identify patient needs such as home health care, DME, physical therapy, and community services
  • Identify gaps or barriers in treatment plans to ensure upon discharge, that patients receive appropriate resources and services
  • Communicate with providers and multi-disciplinary care teams to facilitate the transition of services throughout the continuum of care when applicable
  • Respond to clinical requests by providing accurate information promptly
  • Maintains clinic and patient confidentiality
  • Performs other duties as required and assigned by Manager, including schedule changes and travel to office locations as assigned
