Schedule: Day, Monday- Friday 8am to 430pm
SUMMARY:
As an Occupational Therapist, your strong attention to detail and excellent evaluation skills are crucial to successfully restoring or improving each patients optimal level of function, independence and quality of life.
As the only program of its kind in the region, ElderONE provides all the medical, emotional, and social support seniors need, while still allowing them to live at home. We bring together scheduling, transportation, and all-inclusive care so seniors and their loved ones can enjoy peace of mind.
RESPONSIBILITIES:
- Evaluation & Development. Perform a complete initial evaluation of referred patients using evaluation protocols, standardized tests and clinical observations; develop and implement treatment plans, including short- and long-term goals and methods to achieve them; adjust treatment plan and goals as needed
- Documentation & Communication. Maintain complete documentation of patient evaluation, treatment, progress and discharge status; counsel and teach patients and families; coordinate care plans with other involved health professionals and discuss changes in treatment programs with MDs
- Employee Development. Assist with the supervision and guidance of new personnel, assistants, students and volunteers as assigned; develop and conduct in-service training programs
REQUIRED QUALIFICATIONS:
- Degree in Occupational Therapy, bachelors, masters or doctorate.
- Current NYS Occupational Therapist License or limited permit as a Registered Occupational Therapist.
PREFERRED QUALIFICATIONS:
- 3 to 6 months of previous experience preferred.
- ElderONE EMPLOYMENT: 1 year experience with frail elderly population preferred

M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.For disease specific care programs refer to the program specific requirements of the department for further specifications on experience and educational expectations, including continuing education requirements.

It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
