Position Summary

The Clinical Case Manager delivers comprehensive community-based support services to assist clients in addressing immediate needs and enhancing their overall treatment outcomes. This role involves active outreach, connecting clients with appropriate services, resources, and support systems. The Case Manager collaborates across departments and service sites, facilitating access to care and ensuring continuity of support. They provide clients with guidance and education on community resources, actively assist them in navigating necessary tasks, and ensure all interventions are aligned with individualized treatment plans. Ongoing communication with clinical and support staff ensures coordinated and effective service delivery.

PRIMARY ACCOUNTABILITIES

Primary Accountabilities

  • Achieve Results and measurable outcomes according to established goals.
  • Assess client needs and connect them with appropriate services and resources, providing clear rationale for referrals.
  • Address and resolve immediate client issues to support the success of individualized treatment goals.
  • Perform and document biopsychosocial assessments according to Sam Rodgers policies and procedures.
  • Develop, coordinate and monitor a service plan as well as evaluate plan effectiveness.
  • Assist clients in completing tasks aligned with their treatment plans and agency objectives.
  • Collaborate with internal and external resources to ensure clients receive effective and timely services.
  • Maintain accurate tracking systems (e.g., logs or databases) for all client referrals and care management activities.
  • Prepare thorough documentation and reports to support continuity of care and monitor client progress.
  • Record relevant clinical and case management information to support treatment outcomes.

Operational Excellence

  • Conduct all duties in alignment with the organization's mission, demonstrating fiscal responsibility and upholding its reputation and values.
  • Ensure compliance with treatment program goals, grant guidelines, laws, regulations, and industry best practices.

Relationships

  • Foster positive working relationships with internal teams, including co-workers and other departments, to support client success.
  • Collaborate closely with agency staff to provide integrated, client-centered care across services.
  • Develop and maintain strong external partnerships with clients, vendors, funding sources, and key community stakeholders.
  • Keep up-to-date information on frequently utilized community agencies to support program operations and referrals.
  • Actively participate as a member of the multidisciplinary Behavioral Health Services team.
  • Establish liaison relationships with external agencies to streamline referrals and strengthen inter-agency collaboration.

Qualifications

POSITION REQUIREMENTS

Experience

  • Needs to have demonstrated success in providing support to clients with psychological and other clinical or social issues.

Education

  • Meets minimum of an Associate degree, with a Bachelors degree being preferred.
