Physical Therapist (Part Time) - PACE/Elderplace

This is a part time position supporting our Laurelhurst site.
This position is responsible for PT evaluations and development and implementation of the PT plan of care in accordance with PACE policies. The PT is a member of the Interdisciplinary Team and responsible to communicate and coordinate care with the team as well as multiple outside providers, caregivers and family members. The PT functions independently, working in multiple settings including the PACE center, participant homes, Adult Family Homes, Assistive Living Facilities and nursing homes. The PT assumes responsibility for participants treated by the PTA and PT aide.

Required Qualifications:
- Bachelor's Degree in Physical Therapy.
- Oregon Physical Therapist License upon hire.
- National Provider BLS - American Heart Association upon hire.
Preferred Qualifications:
- Master's Degree in Physical Therapy. Or
- D.P.T. in Physical Therapy.
- One (1) year working with a geriatric population.
