Position Description:
The Case Manager is the central force of both the internal and external treatment team. The Case Manager obtains, provides, and utilizes all clinical information about the client to develop the treatment plan, present the clients status, and communicate about the treatment process. The Case Manager is the primary liaison between all parties involved with the clients case. As a Case Manager you will work in a team environment that provides opportunity to learn from others and build a solid understanding of treatment strategies.

Position Description:
The Case Manager is the central force of both the internal and external treatment team. As a Case Manager you will work in a team environment that provides opportunity to learn from others and build a solid understanding of treatment strategies.
Responsibilities:
- Administer Northwest Passages programming, centering on blending sophisticated mental health treatment with experiential and expressive opportunities to help children and adolescents find ways past lifes struggles.
- Communicate with treatment team regarding clinical and educational needs of the client and facilitate the development and implementation of a treatment plan that addresses those needs.
- Regularly communicate with the clients support network and supporting treatment teams regarding the goals of the program, initial assessments, treatment planning and reviews, and the rights and responsibilities of clients and their families.
- Facilitate family participation in the program, treatment plan, and discharge plan as appropriate.
- Effectively facilitate interagency meetings as a representative of the program and treatment team including ongoing reviews of client progress and treatment planning, facilitating discharge and aftercare planning, and mediating, if necessary, any conflict between the client and/or his or her family and the program.
- Serve as an advocate for clients and their families while maintaining a positive relationship with the community and other networking groups.
- Complete necessary documentation as required by certification, funding source, and program standards.
- Maintain an understanding of mental health concepts, psychotropic medications, and client symptomology.
- Other duties as assigned.
Qualifications:
- Bachelors degree in a social or behavioral science field.
- Communication and interpersonal skills to work effectively with clients, staff, and others.
- Strong problem-solving, and observation skills.
- Ability to manage multiple projects simultaneously and meet various deadlines.
