Position Overview
Reporting to the Home Care Director, the Granite VNA Occupational Therapist (OT) delivers age appropriate care to the patient in the home setting utilizing standard therapy practice: assessment, planning, intervention, implementation and evaluation within scope of practice defined by the NH Occupational Therapy Board. OTs will effectively interact with patients, caregivers and other healthcare team members while maintaining all standards of professional therapy. OTs will supervise Certified Occupational Therapist Assistants assigned to patients.
Essential Functions

- Develops and implements appropriate plan of care (POC) and discharge goals in partnership with the patient, patient representative (if any) and/or caregiver(s) based on thorough assessment incorporating all aspects of patient limitations and goals, and potential discharge environment
- Documents accurate and ongoing assessment, treatment and discharge of the patient. Documentation includes evidence of progress towards goals, appropriate patient/caregiver teaching, and the understanding of these instructions in accordance with agency timeframes.
- Develops and implement POC that comply with referral source orders, include specific treatment measures and indicate frequency and expected duration as indicated on the POC.
- Engages the patient /caregiver in meaningful education as indicated per care plan.
- Communicates with all ordering providers involved in the POC and other health practitioners related to the current POC.
- Demonstrates responsibility to effectively coordinate the patients POC with members of the interdisciplinary team and other health professionals through team/patient care conference. Updates patients short-term/long-term goals as appropriate based on interdisciplinary team assessments and re-assessments.
- Participates in interdisciplinary team, agency projects, committees, QAPI program and/or other activities upon request and approval from direct supervisor. Attends Agency In-service programs.
Other Job Functions
- Supervise Certified Occupational Therapy Assistants assigned to patients according to federal and state regulations.
- Coach, mentor and precept new occupational therapy employees and students.
- Performs job duties in a safe manner in order to prevent injury to yourself and others.
This is a full time position Monday - Friday 8:00 am - 4:00 pm
Work assignments include holiday, weekend rotation.
RequirementsMinimum Qualifications and Requirements
- Graduate of an approved occupational therapy program, accompanied by current New Hampshire occupational therapy license.
- One year of occupational therapy experience.
