Qualifications

About the Role: Medical Team: Case Manager
As a Case Manager with the North Range Behavioral Health Medical Team, you will be the bridge between clients and the services that can change their lives. You will work closely with therapists, psychiatrists, medical providers, and community partners to connect people with the right resources, monitor progress, and help them reach their treatment goals. This is meaningful work that blends advocacy, problem-solving, and compassionate care.

Key Responsibilities:

  • Support clients directly – carry a caseload and work one-on-one to help clients follow their treatment plans and achieve their goals.
  • Collaborate with care teams – partner with therapists, psychiatrists, and other staff to coordinate services, address needs, and prevent crises or hospitalizations.
  • Guide care transitions – help clients move between different levels of care, from intensive therapy to medication management (and vice versa), ensuring the right level of support.
  • Advocate and connect – link clients to community resources, medical care, housing, education, and other supports.
  • Monitor progress – track treatment goals, maintain detailed documentation, and celebrate client successes.
  • Partner with the community – work with agencies, landlords, banks, and other partners to remove barriers and provide stability.
  • Support our medical team – assist with pharmacy coordination, letter preparation, and other special requests.

Qualifications:

  • A bachelor’s degree in a human services field from an accredited college or university is required.
  • A Valid Driver’s License and driving record that meets agency insurability requirements are required.
  • Bilingual Skills (English/Spanish) desired
  • Successful completion of background, fingerprinting, and drug testing

Experience:

  • No prior experience is required, though it’s preferred.

Additional Eligibility Requirements:

  • Annual Flu Vaccinations
  • Annual TB Testing
