Occupational Therapist Skilled Nursing Facility & K-12 School Setting
Location: Provo, UT
Duration: 12-week Contract | Full-Time (40 hours/week, Monday-Friday) | School Year 2025-2026
This position offers a unique blend of opportunities within both educational and healthcare environments. The primary assignment is in a skilled nursing facility providing long-term care, post-acute rehabilitation, and geriatric outpatient therapy services. In addition, there will be assignments in a K-12 school setting, serving as a vital resource in supporting occupational therapy needs for students.
Responsibilities include:
- Evaluating adult and geriatric patients and students, creating individualized treatment plans
- Performing progress reports, recertifications, and discharge planning
- Managing a caseload predominantly at one main location, with occasional assignments at up to three locations, including outpatient services
- Ensuring timely and accurate documentation to support billing and care continuity
- Collaborating with interdisciplinary teams to optimize patient and student outcomes
Desired qualifications and experience:
- Valid Utah Occupational Therapy license
- Experience in skilled nursing facilities (SNF), particularly with long-term and post-acute care
- Familiarity with electronic health records, specifically NetHealth and PCC systems
- Background working in school-based occupational therapy is highly valuable
- Strong clinical evaluation and documentation skills
