Duties:

1. Primary General Responsibility

a. CTT Case Manager to conduct three psycho-educational interventions with the client every six months resulting in the enhancement of communication, boundaries, and/or emotional well-being of at least 80% of the clients served based on survey and self-report

b. CTT Case Manager to connect client to at least two community based resources every 90 days resulting in a noted increased stability of at least 60% of clients served based on survey and self-report.

2. Second general responsibility.

a. CTT Case Manager to conduct three visits weekly resulting in at least 120 contacts monthly.

b. CTT Case Manager to engage with the client in the community and/or home in at least 75% of visits.

Any other duties may be assigned as deemed appropriate.

Job Purpose/Summary:

The Continuous Treatment Team (CTT) case manager serves as a clinical member of the multi-disciplinary team, providing treatment, rehabilitation, and support for SPMI client population. CTT case managers may provide direct coverage Monday through Friday. Provides information and referral, agency linkage, advocacy, home visits and follow-up services as defined by the Case Management Service Plan. Provides clinically appropriate transition to clients transitioning out of the CTT program. Provides crisis intervention and emergency services as needed. Utilizes center wide and community services as needed for client benefit. Embraces the key values of case management: empowerment, normalization, rehabilitation, and continuity of care.

Summary of position : This person will provide primary case-management for an assigned group of clients, including on-going assessment, rehabilitation and support services. Must have a strong commitment to the SPMI population where the goal is to help the client maintain within their natural environment, home, work, and community, and to learn to access appropriate support services.

Works in a multi-disciplinary team approach to meet clinical needs of SPMI consumers.

Attends and is timely to weekly multi-disciplinary Team Mtgs.

Attends administrative team meeting on a weekly basis without tardiness.

Responds to all flags, emails and voicemails within 2 business days.

Provides coverage as needed for like programming.

Completes all required documentation in compliance with Center Policy, CARF and SSOC standards.

Completes treatment plan within 30 days of admission with input from other professionals as evidenced by signatures of MD, RN and Social Worker.

Completes Crisis Plan, DLA-20, and treatment plan within 30 days of ct. admission date.

Completes No Harm contract at ct.'s first sign or report of SI, HI, or thoughts of harmful behavior.

Ensures appropriate referral paperwork is completed within 2 days of post-discharge as evidence by documentation in MR.

Completes all pre-certs or concurrent review paper forms at least five business days prior to deadline so document can be submitted to UM for review.

Completes progress notes within one week of encounter.

Completes ROI for client's PCP and other active providers (day care, school, therapists, P.O, etc?), updating them every 6 months as needed.

Conducts post-hospitalization contact with clients within 7 days of discharge.

Engages clients and their families in the treatment process providing treatment planning, linkage, referral, and advocacy for each client.

Conducts 10-14 hours of face-to-face contacts on each client on their caseload per month with a monthly productivity goal of 120.

Creates treatment plan with goals based on ct.'s input as well as input from family and other systems within 30 days of being admitted.

Creates and implements at least 1 measurable treatment intervention per treatment plan goal.

Utilizes family strengths and empowers the family to implement effective interventions as evidenced by the client's progress toward goals.

Makes referral to less intensive services upon anticipation of d/c.

Cm works with the ct. for up to 90 days post d/c to ensure transition has been successful.

Typical Working Environment:

This position is based out of an office location but requires face to face encounters in the home and engaged with the client. Treatment Team and primarily administrative functions occur in office while clinical interventions and case management duties are executed in the home and community.

Compensation:

  • Starting salary for this position is approximately $19.34/ hr based on relevant experience and education.

Schedule:

Ability to complete all essential job duties within the 40 hour work week. Ability to respond to call from clients via phone within 30 minutes. Ability to schedule and carry 10 to 14 contacts with each client monthly. This position is in person counseling with up to 14 face to face contacts per month.

Travel :

Ability to travel and properly document mileage. Ability to assist clients with travel and participate in client transports utilizing personal vehicle. This position also requires that the employee has an F Endorsement on his/her Driver License to be available to transport individuals as needed.

Equipment/Technical Competency :

Must also have the ability to communicate effectively and possess good time management and organizational skills.

Must be computer literate and have ability to communicate effectively (oral and written communication.)

Equipment/Technology:

Ability to effectively document in electronic medical records (EMR) duration, assessments, interventions, and participates in each encounter in order to demonstrate effective treatment course. Use of a computer is required and the individual must exercise competency in electronic record keeping. Competency in Excel and Word is preferred. Use of work cell phone is required.

Qualifications -Hamblen Adult CTT Case Manager

Experience / Knowledge:

Must have course work and or experience in the areas of cultural diversity, human development, etiology and treatment of mental illness, alcohol and drug abuse, physical and sexual abuse, suicide, and mental retardation. One-year experience preferred. Preferred: experience in developing treatment plans, implementation of treatment plans for adults and/or families; maintenance of casework documentation and progress notes; serving as liaison between agency and community partners such as schools, courts, DCS etc.; therapeutic support to adults regarding educational goals, anger control, grief issues, separation issues, and other personal/family issues; crisis intervention and transportation of adults. CTT experience preferred.

Education / License :

Bachelor's degree in a health-related field of counseling, psychology or social work. Candidates with a Bachelor's degree in rehabilitation, occupational therapy, criminal justice, and education must have fifteen (15) college-level semester hours of course work in behavioral health and at least one (1) year of work experience in the behavioral health setting. This position also requires that the employee have an F Endorsement on his/her Driver License to be available to transport individuals as needed.

Physical/Emotional/Social - Skills/Abilities:

Ability to effectively and ethically counsel children and families. Ability to present professionally and work within a team format to plan, implement, and evaluate successful interventions. Ability to engage the client. Ability to work within a team format to meet positive goals for adults and interface with other agencies involved in the ecology of the family. Required to be certified in and adequately implement verbal and physical de-escalation techniques that include a wide range of bodily movements including but not limited to grasping, holding another person, running, and walking. Physical de-escalation techniques will only be implemented at Helen Ross McNabb Center facilities, and not in community settings.

Must have mental ability to exercise sound judgment under pressure.

Ability to exercise effective decision-making, ability to set and demonstrate appropriate boundaries, ability to be an empathic listener, flexibility, willingness, and adaptability to working with diverse populations. Must also have the ability to communicate effectively and possess good time management and organizational skills. Demonstrated ability to abide by professional/ethical code of conduct and to establish supportive and respectful relationships with clients. Hearing of normal and soft tones. Close eye work. Lifting up to 50 pounds. Frequent sitting, standing, walking, bending, stooping, and reaching. Must have an F endorsement and a valid Driver's License. This position requires utilizing a personal dependable vehicle to conduct Center business. Maintaining a dependable vehicle and certified driver status is a condition of employment.
