Job Description:
Sunbelt Staffing is seeking a dedicated Home Health Physical Therapist for a 13-week assignment in Longmont, CO, with the possibility of transitioning to a permanent role. This position involves managing a caseload of approximately 27 patients per week, with per-visit rates, and traveling between patient homes. Documentation turnaround is required within 24 hours for SOC and 48 hours for other visits. The facility uses MatrixCare and provides a supportive orientation of 23 days, including shadowing and additional training if needed.

Key Responsibilities:

• Evaluate, plan, and implement patient-specific physical therapy treatment plans

• Conduct SOC, follow-up, and discharge visits according to home health protocols

• Maintain accurate and timely documentation in MatrixCare

• Collaborate with the interdisciplinary team to ensure coordinated patient care

• Travel between patient homes in the Longmont area, providing high-quality therapy services

Qualifications:

• Current Colorado Physical Therapist license

• Current BLS certification

• Minimum 6 months of home health experience preferred

• Proficiency in MatrixCare required

• Reliable vehicle and valid drivers license

• Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission
