Job Opening: Patient Care Coordinator (RN or LMSW)
Location: Meadowlark Hospice | Full-Time
Reports to: Program Director
Meadowlark Hospice is seeking a compassionate and dedicated Registered Nurse (RN) or Licensed Master Social Worker to join our team as a Patient Care Coordinator. This is a full-time supervisory position that plays a key leadership role in the coordination and delivery of high-quality hospice care.
Position Summary:
The Patient Care Coordinator is responsible for the management and supervision of patient care services, as well as the utilization of community resources to support patients and families. The role includes oversight of clinical staff, ensuring seamless care delivery, compliance, and effective communication with referral sources and partner facilities.
Key Responsibilities:
Supervise clinical staff by discipline
Manage patient referrals and admissions
Coordinate daily staff assignments and call schedules
Conduct staff evaluations and lead in-service training
Oversee charting, documentation, and regulatory compliance
Maintain communication with facilities, providers, and referral sources
Provide direct patient care as needed, including coverage for visits and on-call shifts for nursing
Schedule:
Monday through Friday
Participation in on-call as needed for on call questions
Qualifications:
Current RN license or Licensed Masters Social Worker in the state of Kansas (or ability to obtain)
Hospice experience preferred, but willing to train the right candidate
Strong leadership and communication skills
Ability to work collaboratively in a team environment
Reliable, organized, and compassionate
